At Collingwood Dentistry, we believe that quality dental care should be accessible to everyone. Our Dental Payment Plan is designed to help uninsured patients receive the care they need with manageable payment options.

The payment plan covers a wide range of essential dental care services, including but not limited to:

Services Covered

• 2 Routine cleanings

• Basic and comprehensive exams

• X-rays

• Preventive care, such as fluoride treatments

Note: Specialized treatments may not qualify for this plan. Contact us for a full list of covered services.

Eligibility Requirements

To enroll in our payment plan, patients must:


  • Be a new or existing patient at our dental practice.
  • Must not have any dental insurance coverage.
  • Be able to provide a valid Credit Card for automatic monthly withdrawals.
  • Be 18 years or older or have a legal guardian co-sign.
  • Agree to a credit check to assess payment reliability.
  • Must be used within 1-year of enrolment.
  • No-shows or late cancellations incur a $50.00 fee.

Payment Terms

Down Payment: A $99.00 down payment of the total treatment cost is required at the time of enrollment.

Monthly Installments: Payments must be made monthly via automatic withdrawal from a valid Credit Card.


Late Payment Fee: A $25 fee will be applied to payments made after a 7-day grace period.

Missed Payments: The plan will be void if two consecutive payments are missed, and outstanding balances will become immediately due.

Cancellation Policy

Patients may cancel the plan with a 30-day written notice. However, refunds are not issued for treatments already received. The dental practice reserves the right to cancel the plan if payment terms are violated, including late or missed payments. Upon cancellation, any unused portion of the down payment may be refunded, minus the cost of services already rendered. Outstanding balances at the time of cancellation will be immediately due and subject to applicable interest and late fees.

Dispute Resolution

Any concerns or disputes should first be addressed by contacting our financial coordinator at 1-705-293-7700 If unresolved, patients may submit a formal written complaint to our practice manager. The practice manager will investigate and respond in writing within 14 business days. If necessary, independent mediation can be arranged at the expense of the patient. Legal action is a last resort and will adhere to the jurisdiction’s governing laws.

Communication Guidlines

We are committed to providing transparency regarding the plan. To ensure patients understand the terms and conditions, we will:

  • Provide a detailed brochure outlining the payment plan guidelines.
  • Offer a consultation with a financial coordinator to discuss the plan in detail.
  • Include the payment plan information on our website under “Payment Options.”
  • Have patients sign a consent form acknowledging that they have read and agree to the terms.
  • Send a confirmation email detailing the payment plan after enrollment.
